Why we ask: date of birth to check SSI age limits and register your training with SSI, and shirt and shoe sizes so the right wetsuit and fins are ready. If you're giving details for other adults, please let them know and point them to our privacy policy. This form goes through Cloudflare to our Gmail inbox, which can store it overseas, including in the United States. You can tell Mim on the phone instead. No health questions here.

If the person taking part may not be able to understand our risk warning, because of their age or a disability, a parent, guardian or carer must read it for them before the session. For an adult participant, that person must also come with them to the session and be at the briefing. They don't have to watch the session.
